Kirbyville R-VI School Foundation 5K

I understand that participating in a 5K race is a potentially hazardous activity. I assume all risks from participating in this event and its
related activities including, but not limited to: falls, injuries, contact with other participants, effects of weather and conditions of the
trail. I acknowledge I have been advised of all such risks and they are known to me. I, for myself and anyone entitled to act on my
behalf, waive and release and agree to indemnify the Kirbyville R-VI School District (including all school board members and employees),
Kirbyville R-VI School Foundation, Empire District Electric Company, race officials, volunteers, sponsors and their affiliates
from all claims, liabilities or causes of action of any kind arising from my participation in this event or related activities. The release
and waiver applies to all claims of every kind and nature and shall be construed in the broadest sense allowed by law. I grant permission
to use any photographs of myself or my child on the Kirbyville R-VI District website, Kirbyville R-VI Foundation web page, and
on other web pages, video or print media designed to document or promote this race or future races. Applications for minors (under
18) will be accepted only with the signature of a parent or legal guardian. All children under the age of 16 must be accompanied by an
adult at the race. By signing below, I specify that I am in acceptable physical condition and medical condition to participate in this
event and understand and agree to the above waiver and agreement to indemnify.
