I know that running or volunteering for a fun run/walk is a potentially hazardous activity and could cause injury or death. I will notenter and participate unless I am medically able and properly trained. By my signature, I certify that I am medically able to performthis run/walk activity, am in good health and am properly trained. I agree to abide by any decision of a run/walk volunteerrelative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation forany reason whatsoever. I attest that I have read the rules of the run/walk and agree to abide by them. I assume all risks associatedwith running or walking in this event, including but not limited to: falls, contact with other participants, the effects of theweather, including cold, high heat and/or humidity, traffic and the conditions of the road and trail, heart attacks or other typesof potentially hazardous outcomed including death are possible during involvement in such an activity. I understand that bicycles,skateboards, roller skates or roller blades, animals and personal music players are not allowed in the run/walk and I will abide byall run/walk rules. Having read this waiver and knowing these facts and in consideration of your accepting my entry, I, for myselfand anyone entitled to act on my behalf, waive and release the American Cancer Society, Inc., Festival of Hope 5K, the city ofSpringfield and all event sponsors, their representatives and successors from all claims or liabilities of any kind arising out of myparticipation in this event. I grant permission to all of the above to use my photographs, motion pictures, recordings or any otherrecord of this event for any legitimate purpose.ActNow Promotions Waiver: I understand running is a potentially hazardous activity. Having read this waiver and knowing these facts, and considering your accepting my entry into this running event, I, for myself and my estate, heirs, survivors, descendants and anyone on whose behalf I may act (including all minor children and charges and their estates heirs and survivors), do hereby permanently and forever completely give up, surrender, waive, release and terminate any and all claims or liabilities of any kind, arising in tort, contract or otherwise, against ActNow Promotions/Racing, and their respective officials, directors, trustees, agents, volunteers, employees, sponsors and affiliates arising from my participation in this event, including but not limited to claims arising out of past or future negligence or carelessness on their part.
