Covering: 2018 Happy Feet Run & Walk for Republic Pregnancy Resource Center (Event).I, , understand that my, or my minor child, or the minor child in my legal custody's participation in theEvent is at my own risk. I understand there are risks associated with physical activity involved in the Event, includingbut not limited to possible falls, contact with other participants, effects of weather, traffic, and road conditions.I understand that, in the eventthat (participant’s name) requires medical or dental treatmentwhile engaged in the activity, reasonable efforts will be made to contact me; however, if I cannot bereached, I hereby consent and give my permission to the Event’s sponsor or any adult counselor acting on behalfof the ministry with respect to the Activity, as agent for me, to consent to any X-ray examination, injections, anesthesia,medical, dental, or surgical diagnosis and treatment; and hospital care and treatment advised and supervisedby a physician, surgeon, or dentist licensed to practice under the laws of the state where the services arerendered, either as on outpatient or in any hospital.I understand that my insurance coverage for myself or my child will be used as primary coverage in theevent medical intervention is needed. Coverage by Calvary Baptist through its accident policy may be availableas a backup for what my family’s insurance does not cover.I understand all reasonable safety precautions will be taken at all times by Calvary Baptist and RPRC andits agents during the events and activities. I understand the possibility of unforeseen hazards and know the inherentpossibility of risk. I also agree not to hold Calvary Baptist, RPRC, leaders, employees, and volunteer staffliable for damages, losses, diseases, or injuries incurred during participation in the activities and events.I also understand that visual and audio recordings may be made of these activities, and can be used by asneeded for publicity, announcements, scrapbooks, etc.I also recognize that by signing this waiver there is no right to a refund if I am unable to attend the race.ActNow Waiver:ActNow Promotions Waiver: I understand running is a potentially hazardous activity. I should not enter and participate unless I am physically fit, mentally able and properly trained. I agree to abide by any decision of the race officials relative to my ability to safely complete the run. I certify I am in good health and properly trained to run the distance of this race. I assume all risks associated with running in this event, all risks being known and being appreciated by me. I understand my entry fee is non-refundable. Having read this waiver and knowing these facts, and considering your accepting my entry into this running event, I, for myself and my estate, heirs, survivors, descendants and anyone on whose behalf I may act (including all minor children and charges and their estates heirs and survivors), do hereby permanently and forever completely give up, surrender, waive, release and terminate any and all claims or liabilities of any kind, arising in tort, contract or otherwise, against ActNow Promotions/Racing, and their respective officials, directors, trustees, agents, volunteers, employees, sponsors and affiliates arising from my participation in this event, including but not limited to claims arising out of past or future negligence or carelessness on their part. I hereby consent to the use of image in photographs, videos and other recordings for promotional purposes.Registration fees are nonrefundable.
