Shelby's Run #8

FOR ALL PARTICIPANTS IN THE SHELBY'S RUN RACES AND RELATED EVENTS (SRR) ARE REQUIRED TO ASSUME ALL RISK OF PARTICIPATION IN THE SRR BY SIGNING THIS GENERAL RELEASE AGREEMENT.
The undersigned athlete (Athlete) on behalf of himself/herself and on behalf of Athlete's personal representatives,
assigns, heirs, executors, hereby fully and forever releases, waives, discharges and covenants not to sue the City of Clever, Clever R-V School District, SRR, St. Jude Children's Research Hospital (St. Jude), and all public or private agencies whose property and/or personnel are used and all other sponsoring or co-sponsoring companies or individuals,
including volunteers, related to the SRR (collectively releases) from all liability of any kind to the Athlete and his/her personal representatives, assigns, heirs and executors, for all loss(es) or damage(s) and any and all claims, demands, losses or damages on account of injury or death to the Athlete or property or resulting in the death of the Athlete, whether caused by the active or passive, foreseeable, or contributory negligence of all or any of the releasees or other-wise, in connection with the Athlete's participation in the SRR. Athlete acknowledges and agrees that the organizers of the SRR, in their sole discretion, may delay or cancel the SRR or related events for any reason. There shall be no refund of the entry fee or any other costs of Athlete in connection with the SRR. The Athlete represents and warrants that he/she is in good physical condition and is able to safely participate in the SRR. The Athlete is fully aware of the risks and hazards inherent in participating in the SRR and elects to voluntarily compete in the SRR, knowing the risks associated with the SRR. The Athlete hereby assumes all risks of loss, damage, or injury that may be sustained by him/her while participating in the SRR.
While this is not a USATF sanctioned event, Rule 144.3b which prohibits the use of headphones and other music or
program-listening devices while on the race course will be observed.
The Athlete agrees to the use of his/her name and photograph in broadcasts, newspapers, brochures and other media without compensation. The Athlete acknowledges that the entry fee is non-refundable and non-transferable. The Athlete hereby grants to SRR and their agents, affiliates and designees access to all medical records (and physicians) as needed and authorize medical treatment as needed. The Athlete warrants that all statements made herein are true and correct and understands that releasees have relied on them in allowing Athlete to participate in the SRR.
IF THE ATHLETE IS UNDER AGE 18: I , the undersigned parent/guardian agrees that my son/daughter has my
permission to participate in the SRR; that the parent/guardian has read the forgoing GENERAL RELEASE AGREEMENT and by signing below intentionally and voluntarily agrees to its terms and conditions. The parent/guardian further represents that my son/daughter is in good physical condition and is able to safely participate in the SRR. I hereby authorize medical treatment for him/her and grant access to my child's medical records as necessary. You must be at least 16 years of age to participate in the 10K and at least 11 years of age to participate in the 5K. There is no minimum age to participate in the Shelby's Run Races 1-Mile Fun run, though SRR organizers require that participants under the age of 11 be accompanied by an adult. The SRR reserves the right to require proof of age. Parent or guardian must also sign below for the entrants under 18 years of age. By signing, parent agrees to the same conditions required of Athlete.
ATHLETE HAS READ THE FOREGOING AND INTENTIONALLY AND VOLUNTARILY ACCEPTS THIS GENERAL RELEASE
